How to Prepare to Return Home After a Hospitalization, Presented by BrightStar Care of Salt Lake City

What Do Patients and Caregivers Think is Most Important When Transitioning from Hospital
to Home?

If you are scheduled for surgery or a medical procedure, you may be wondering what the first few days at home will be like when you are recovering. Patients also ask what will make returning home difficult, and what will make the transition easier.

According to a study by the Patient-Centered Outcomes Research Institute, patient and caregiver participants identified the outcomes necessary for a safe and manageable care transition home from the hospital:

  • feeling cared for and cared about during the care transition

  • relying on unambiguous accountability from the health care system, including an understanding of which physician was responsible for overseeing the care

  • feeling prepared to execute the care plan upon discharge

Researchers found patients and caregivers had the following expectations from their discharge planner:

  • anticipating patient and caregiver needs to succeed in self-care at home

  • engaging in discharge planning in collaboration with others

  • giving actionable information

  • providing continuous care until the patient's recovery

BrightStar Care of Salt Lake City works closely with discharge planners, physician offices, and social worker to ensure continuation of care at .home after hospitalization.
