Hospital To Home - Understanding Your Discharge Plan, Presented by BrightStar Care of Phoenix / Tempe

Before Leaving the Hospital, a Phoenix or Tempe Social Worker Will Assist With Your Discharge Plan


A discharge plan will establish the care needed at home, and help prevent readmission to the hospital.


The plan should list the following provisions for your recovery at home:

  • Personal care, such as bathing, eating, dressing, and toileting
  • ​Household care, such as cooking, cleaning, laundry, and shopping
  • Health care, such as making appointments, managing medicines, and using medical equipment
You may require special equipment to be in place before you return home. Your social worker will have resources available to make sure you have everything you need.

Your social worker will also have a list of instructions for you to follow after you leave the hospital: 

  • A description of your medical conditions, including any allergies
  • A list of all your medications and how to manage them. Highlight any new medicines and any that need to be stopped or changed
  • How and when to change bandages and dressings
  • Dates and times of medical appointments and the providers names and phone numbers 
  • Who to call if you have questions, problems, or an emergency
  • What you can and cannot eat. Do you need any special foods?
  • How active you can be. Can you climb stairs and carry things?
Following your discharge plan can help you recover successfully and prevent any future problems.
