1. Resume
2. CPR card
3. TB-PPD or chest x-ray
4. Physical/health certificate w/physician signature
5. 2 proofs of ID (Social Security, Permanent Resident, Tax ID, Passport, etc.)
6. Driver’s license & auto insurance (non-drivers are accepted for live-in positions)
