Job description: RN Case Manager
Job Type: Full-time
Salary: $82,000 – $87,000 per year. COMP negotiable
(Focused on finding the right person to join our growing Home Care Agency)
Expected hours: M-F, 9am-5pm (some flexibility)
The RN Case Manager is responsible for the overall direction of home health clinical services for skilled and non-skilled care. The RN Case Manager will have overall responsibility for the quality, adequacy of services provided and supervision of staff, alongside the Director of Nursing. The RN Clinical Case Manager establishes, implements, and evaluates goals and objectives for home health services that meet and promote the standards of quality.

Responsibilities of RN Case Manager:
- Lead, inspire and motivate others to provide exceptional care.
- Visit our adult clients and conduct assessments.
- Develop the care plan and oversee its execution.
- Supervise field staff (Nurses, HHAs, CNAs, etc).
- Facilitate new hire orientation.
- Participate in the training of new nurses and in-services for field staff.
- Wound management
- Venous puncture
- Medication management
- Ostomy care and experience
- Catheter care and experience
- Tracheostomy care and experience
- IV infusion experience including the ability to perform all types of IV administration and insertion of a peripheral IV
Skills also desired include:
- Ability to function as a team player as well as independently when needed
- Ability to be a leader and manage time appropriately
- Ability to handle situations with tact and ease
- Excellent independent assessment skills
- Previous experience with home health care
- Good customer service experience

Qualifications for RN Case Nurse Manager:
- Valid RN license in the state of Pennsylvania
- Bachelor's degree in nursing (BSN) preferred
- Minimum three (3) years of clinical experience preferred.
- Reliable car/auto insurance
- Clean background and criminal record.
- Understands regulatory compliance with state and federal regulations.
- Adhere to HIPAA and maintain client confidentiality.
- Demonstrated ability to supervise and direct staff.
- Possess exceptional nursing assessment skills; Analytic/Critical thinker.
- Proven ability to build quality and implement improvements within the operation.
- Policy development and case management experience is a plus.
- Strong knowledge of medical procedures and protocols
- Excellent communication and interpersonal skills
- Ability to multitask and prioritize responsibilities

Responsibilities of Director of Nursing:
- Provides care as defined within the scope of the state’s nurse practice act.
- Assists in preparation of clinical and progress notes and summaries of care.
- Assists in the development of organization goals. Helps develops, recommends, and administers agency policies and procedures.
- Supervises, educates and coaches other nursing personnel/care-giving staff.
- Plans and implements in-service and training programs.
- Oversees the maintenance of client clinical records, statistics, reports and records for purposes of evaluation and reporting of organization activities
- Continues assurance of proper maintenance of clinical records in compliance with local, state and federal laws.
- Stays informed about changes in the field of nursing and home health care. Shares information with appropriate organization personnel.
- Promotes home health referrals in the health care community.
Requirements of Director of Nursing:
- Current unrestricted RN license in state for which care is being provided.
- Preferred: Bachelor’s degree in Nursing. Must have graduated from a National League of Nursing accredited program.
- Minimum five (5) years of documented experience required.
- Reliable car/auto insurance.
- Understands regulatory compliance with state and federal regulations.
- Adhere to HIPAA and maintain client confidentiality.
