The phlebotomist is a member of the home care team who works under the supervision of a registered nurse. The phlebotomist is responsible for collecting blood specimens from clients and ensuring the integrity of the sample(s). He/she will be assigned in a manner that promotes quality, continuity and safety of a client’s care.
Responsibilities:
The Licensed Practical Nurse LPN / Licensed Vocational Nurse LVN is responsible for providing direct skilled client care under the supervision of a registered nurse. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive client care outcomes.
Qualifications:

The phlebotomist is responsible for collecting blood specimens from clients and ensuring the integrity of the sample(s). He/she will be assigned in a manner that promotes quality, continuity and safety of a client’s care.
Responsibilities:
- Perform phlebotomy as directed by the Director of Nursing or registered nurse supervisor.
- Follow established aseptic blood drawing procedures and techniques.
- Follow principles of infection control and Universal Precautions (i.e. Disposable gloves will be worn for all blood specimen collections.
- Determine appropriate site for obtaining the specimen.
- Insure integrity of specimen to include but not limited to the following:
- Verify identify of client prior to procedure
- Verify appropriate site to be used for procedure
- Obtain blood specimen as ordered
- Verify appropriate medium for specimen collection
- Verify accurate label of specimen
- Ensure accurate storage
- Ensure chain of custody of specimen
- Identifies and participates in performance improvement activities.
- Assist in providing a safe environment and promote quality patient care through adherence to established policies, procedures and standards.
- Ensure client’s rights are adhered to. These include but are not limited to:
- the right to privacy and dignity while care is being provided
- the right to accept or refuse care
- the right to confidentiality of their information
- Respond rapidly and appropriately in an emergency situation
- Use equipment properly and in a safe manner.
- High school diploma or GED
- Successful completion of a phlebotomy training program
- Graduate of an accredited phlebotomy training program preferred
- Minimum of one (1) year of documented experience required
- Certified in CPR
- Negative TB skin test or chest x-ray
- Licensed driver with automobile that is insured in accordance with the organizations requirements.
- Self-directing with the ability to work with little direct supervision
- Empathy for the needs of the client
- Demonstrate effective oral and written communication skills
- Ability to calm and reassure client(s) who are anxious about the procedure
- Ability to express spoken and/or written ideas in English
- Treats clients, staff and the public with courtesy, respect and presents appositive public image.
- Works as a team member
- Ensures confidentiality and security of the client’s medical information

Qualifications:
- Graduated from a National League of Nursing accredited practical or vocational school of nursing.
- Unencumbered license as a Practical or Vocational Nurse in the state that care is delivered.
- 1 year of nursing experience required; three years nursing experience preferred. Community, home health or medical/surgical experience preferred.
- Comply with the state nurse practice act.
- Comply with accepted professional standards of practice.
- High level of interpersonal skills.
- Certified in CPR.
- Strong clinical skills.
- Excellent customer service skills and ability to deal tactfully with clients, other team members and community.
- Treat clients, team members and the public with courtesy, respect and presents a positive public image.
- Empathy for the needs of the client.
- Works as a team member.
