Hospital Discharge Planning for Boulder and Broomfield Patients, Presented by BrightStar Care of Boulder

July 24, 2018

Discharge Planning Should Start as Soon as Possible!

What are your plans for recovering after a hospitalization from an injury or illness?  If you have a planned stay in the hospital, you should start discharge planning before admission.

Discharge planning involves:

  • Assessing the patient’s psychological, social and cultural needs
  • Developing a plan of care after hospitalization
  • Arranging for provision of services
During a hospital stay, patients are cared for by a team of doctors, nurses, and other skilled professionals. Another important member of this team is the case manager.  Every patient admitted to the hospital has a case manager who assists them in planning for their needs when they leave the hospital.  Depending on the patient’s needs, a discharge planner may involve the services of social workers and home care agencies, such as BrightStar Care.

BrightStar Care of Boulder works closely with social workers and discharge planners in Colorado to ensure continuation of care after a hospitalization or medical procedure.
