Discharge Planning Helps to Improve the Coordination of Services After a Medical Procedure or Hospitalization from Surgery, Injury, or Illness

Discharge planning is a process that aims to ensure that services after hospital discharge will meet the patient’s needs.  It includes the coordination of services and care.  Thorough planning requires anticipation of potential problems by gathering information, resolving potential barriers to discharge, and timely referral to the multidisciplinary team.

If you or a loved one is preparing for surgery, you should plan and coordinate with family members and a multidisciplinary team or care providers, including you doctor, social worker, and home health care agency if needed. This will lead to improved recovery and patient satisfaction.

Working with a medical social worker will help bridge the gap between hospital and home, reduce length of stay in hospital, and minimize unplanned hospital readmission.  Discharge planning is an important part of hospital care. Social workers also help communicate with primary care staff about patient transfer of care and needs.
